Community healthcare clinics, including primary and urgent care clinics, MAT clinics, and sobering centers with a patient in need of additional or continued substance use treatment, please use the form above to begin the referral process.   

​Community Organizations and Street Outreach Teams    

Staff from community behavioral health organizations (excluding clinics), case management programs, and street outreach teams in San Francisco with a patient in need of substance use treatment (first time or continued treatment), please use the form above to begin the referral process.    

​Court

Staff working within San Francisco County courts, jails and judicial system (including pretrial, diversion courts, parole, probation, child welfare, and family treatment court) with a client in need of substance use treatment should use the form above to begin the referral process.  

​Hospital

Hospital staff (including but not limited to medical providers and social workers) with a patient in need of substance use treatment upon hospital discharge, please use the form above to begin the referral process. Limited to clients currently hospitalized in San Francisco or able to demonstrate San Francisco residency.  

​Jail

Legal teams/representatives of an incarcerated client whose release is dependent on acceptance into treatment, use the referral form above to request assessment and pre-approval for residential substance use treatment. Individuals must be in-custody in San Francisco County or able to demonstrate San Francisco residency. If your client is going to be released on own recognizances or released regardless of acceptance into a program, do not use this referral form.

To seek services for yourself or for a family member, significant other/partner, or friend in need of substance use treatment, use the form here to submit a referral.

Review

HR360 Intake department staff will review the referral information and will reach out to the referrer if there are any outstanding questions.

Decision

HR360 will decide regarding placement. Depending on the type of referral, you will get a message after submission to indicate whether to present to 1563 Mission St or will be contacted by the intake team to schedule.

Admission

Client will participate in the intake process at 1563 Mission to be admitted to the program. After our formal admission process, clients will be transported to their facility location by HR360 staff.
